1. 2. Obtain the phone number or address of the district or regional manager of the repair facility, if you have not resolved the problem by speaking with the service manager. You might be asked to make an appointment for a personal interview or phone conference. Be sure to have all your paperwork at your side at the time of the meeting. You will need the mechanic's name and employee number, as well as the name and number for the shop and service manager. Be prepared to state your case and back up any claims with paperwork and physical proof, if asked to do so.


3. File a claim with your local auto repair bureau, such as the California Bureau of automotive repair. You can locate them in the yellow pages or by visiting the site online for directions. Request the necessary complaint forms and they will be sent to you. Fill out the forms and send them to the appropriate mailing address. Make copies of all your documentation. Make sure the copies have legible numbers, dates, times and employee names and numbers. You can include your own personal photographs of the parts or damages, marked and titled accurately as they apply to the repair.


4. Contact the DMV (Department of Motor Vehicles) Safety Consumer Services for your state. Call them during regular business hours or send a letter to their services section. Ask for a complaint report form. When it arrives, fill it out and send it back with all of the information that was requested. You can also download a form from the their website or fill one out online. Make sure you have all of your paperwork information included on the form.
